SUMMARY:

The Case Manager/Career Counselor position is a dual role that assists Blood Tribe Social Development clients in the areas of Income Support and Pre-employment Supports.  The incumbent will conduct Service Needs Determination assessments to determine eligibility for financial support (basic needs) and assist in goal setting and action planning customized to each individual. The CMCC provides support and guidance to the Blood Tribe Social Developments clients to implement the enhanced Service Delivery / Pre- employment model. The model includes Service Needs Determination, Service Plan Development, monitoring and client mentoring conducive to the Income Assistance program.

AREA OF SUPERVISION:

 

  • The incumbent reports directly to the Case Manager/Career Counselor Supervisor.

 

MAJOR DUTIES AND RESPONSIBILITIES:

 

  • Adhere to existing Blood Tribe Administration Personnel Policies and Procedures and any changes that may occur.
  • The CMCC assists individuals to make informed choices to enable them to achieve and sustain their personal employment and career development goals.  The CMCC will support and mentor the client from entrance into the IA system through provision of pre and post- employment supports and must ensure services are provided in accordance with the ESD Case Management Model, as adopted by Blood Tribe.

 

  • Conduct an in-depth intake/assessment to determine (1) clients eligibility for basic needs and (2) Employment Readiness Assessment and (3) a comprehensive review of aptitudes and skills, employment history and work experience.
  • Help clients identify educational requirements and develop training plans. 
  • Develop Action Plans with clear concise steps to meet expectations/goals of client and method of reporting to Case Manager/Career Counselor.
  • Case Management Follow-up – reviews and monitors progress on individual action plans as required and makes any revisions if needed.
  • Establish client support in the areas of career decision-making, job search, skill development and employment retention/maintenance.
  • Support clients in addressing barriers that may impact their path to self-sufficiency.
  • Complete home visits to further assess the economic and social conditions of caseload clients.
  • Make appropriate and timely referrals to other agencies.
  • Provide counseling supports to clients.
  • Complete budget and decision forms explaining details and providing proof of service.
  • Participates in case conferences with other Case Managers and any other parties involved.
  • Help Clients make realistic occupational or employment decisions.
  • Maintain client file by ensuring all pertinent client information such as; social insurance number, Alberta health care number, Indian status card, income tax form, bank statement are on each individual file.
  • Advocate on behalf of clients.
  • Establish necessary career assessment tools to assist clients in making informed career choices.
  • Assist clients with implementing effective employment search strategies, resume/cover letters and developing career portfolios and interview skills.
  • Facilitate workshops.
  • Assist clients in applications to education institutions and employers.
  • Complete Employment Insurance verification on all clients.
  • Market clients to potential employers.
  • Provide post supports in the areas of follow-up, job coaching and employment counseling.
  • Maintain cooperative working relationships with other employees, agencies and employers.
  • Cooperate as a team player.

 

 

NECESSARY KNOWLEDGE, ABILITIES AND SKILLS:

 

  • Working knowledge of case management procedures and processes.
  • Sound relationship and team building skills.
  • Good Research skills.
  • Excellent problem solving skills.
  • Excellent organizational and time management skills.
  • Excellent interpersonal communication skills are essential to provide information, guidance and counseling to individuals.
  • Computer skills in basic keyboarding, Microsoft Word/internet and e-mail.
  • Operates telephone, fax machine, photocopier, and personal computer.
  • Strong knowledge of Blackfoot culture and ability to speak Blackfoot as asset.
  • Transport clientele on occasion to various appointments.

 


Qualifications

NECESSARY TRAINING AND EXPERIENCE:

 

  • The ideal candidate must have a two-year Diploma in the Human Services field or a Social Work Diploma, knowledge of employment practices, familiarity with case management, career counseling and a minimum two years experience in a related field; Or a combination of education and experience, in case management and career counseling with a minimum of three years related experience.
  • Must have a valid Class 5 driver’s license and a reliable mode of transportation.
  • Must submit a current Criminal Record Check
