Blackfoot Resource Hub - Blood Tribe Management
Term (1 Year with possible extension)
GENERAL DESCRIPTION:
The Indigenous Support Specialist will provide a trauma-informed and strength-based approach through a wholistic lens when assisting Blackfoot Resource Hub clients. The position will provide support by assisting clients to navigate the resources available in the City of Lethbridge. The goal of the Indigenous Support Specialist position is to help prevent clients from experiencing homelessness, and work towards achieving and maintaining independence living off-reserve. The Blackfoot Resource Hub Indigenous Support Specialist will provide support to clients through strong advocating skills and by making referrals to appropriate social support(s) and other resources in the City of Lethbridge. The Blackfoot Resource Hub Indigenous Support Specialist will provide a client-centered approach by creating a strong cohesive support system for clients through collaboration and networking with other supporting agencies in the City of Lethbridge. The successful incumbent must have extensive knowledge on social support programs in the City of Lethbridge and understanding to the intersecting issues impacting Urban Indigenous individuals and families. The Indigenous Support Specialist will be responsible for creating and facilitating social support programs at the Blackfoot Resource Hub ones that will enhance our urban Indigenous populations overall health and wellbeing.
ORGANIZATIONAL RELATIONSHIPS:

Reports directly to the Blackfoot Resource Hub Manager
AREA OF SUPERVISION:
MAJOR DUTIES AND RESPONSIBILITIES:

Perform all duties and responsibilities in accordance with the Blood Tribe Administration Policies and Procedures and applicable policies of Blackfoot Resource Hub.

Receive referrals from diverse sources, including self-referrals; Accept referrals as approved by the Manager.

Manage a caseload of individuals and their families
-
Create care plans to help clients achieve independence.
-
Make referrals to other supporting agencies in Lethbridge that will assist clients to achieve overall health and independence.
-
Create and implement a care plan that will best support clients accessing the Blackfoot Resource Hub.
-
Advocate and make necessary appointments for clients.
-
Provide transportation to appointments (house viewings, food bank, counselling, interviews).

Maintain records related to development, implementation, monitoring and amendment of the care plan to enable and support integrated case management
-
Support the individual clients and/or their family in navigating the Lethbridge ICA system of service providers.

Create a support system for your client by recruiting and supervising Community-based Support Services in Lethbridge and on-reserve support where applicable.

Develop and collaborate with other supporting agencies to create programming that will support Blackfoot Resource Hub client’s wellbeing i.e. (I.D Clinic, Filling Taxes, mental health support, addiction support, life skills) etc.

Attend staff meetings, periodic workshops and interagency meetings.

Assist in the orientation and training of new staff, staff of other departments and entities engaged in case management, allied professionals and Community-based Support Services Workers.

Qualifications
NECESSARY TRAINING AND EXPERIENCE:
-
Must possess a Social Work Diploma or a Human Services Diploma or equivalent with 3 years related experience. A combination of education and experience maybe considered.

Must possess a valid class 5 driver’s license and a reliable mode of transportation.
-
Must provide an updated criminal record and vulnerable sector check.
-
Must have knowledge and understanding of Blackfoot culture, protocols and traditions and the impact on the health and well-being of Indigenous individuals and families.
-
Knowledge and understanding to Urban Indigenous issues. Lived experience preferred.
-
Extensive knowledge of Lethbridge social support programs and agencies.
-
Ability to communicate effectively in the Blackfoot language would be considered an asset.
