GENERAL DESCRIPTION:

The Case Manager is a proactive professional who utilizes an integrated sservices approach to support and strengthen the Blood Tribe. The Case Manager works with individuals, their families, departments and entities of the Blood Tribe, as well as allied service providers, to plan, implement, facilitate, coordinate, monitor and adjust individual and family MyCompass Plan. The Case Manager will have a case load of individuals and families as assigned by the Aiyiikakimaop for ChildrenClinical Coordinator.

 

The Case Manager works within “the Aiyiikakimaop for Children Program, a tribal initiative designed to provide residents of the Blood Reserve with access to supports & services comparable to those available to all Albertans and tailored to the needs and culture of the Blood Tribe, by mobilizing and coordinating the necessary human, partnership and financial resources. Priorities of Aiyiikakimaop for Children include:

  • Addressing identified gaps in publicly funded services with respect to residents of the Blood Reserve (Kainai);
  • Ensuring substantive equality in the provision of services to residents to ensure culturally appropriate services and/or to safeguard their best interests; and
  • Integrating and coordinating services from various departments and entities through person and family focused case management. 

 

Supports and services for individuals and their families are planned, developed and coordinated through the Aiyiikakimaop for Childreninitiatives to be:

  • Respectful of human rights (e.g., provide supports & services generally available to Albertans);
  • Customized to respond to the needs and aspirations of individuals and their families.
  • Aligned with Kainaysinni - the values and culture of the Blood Tribe.
  • Coordinated effectively among the Blood Tribe Departments & Entities, consistent with their respective mandates and respecting their established external partnerships (e.g., with funders).
  • Sustainable – financially and in terms of adequate organizational capacity.
  • Inclusive and engaging of individuals and their families.

 

ORGANIZATIONAL RELATIONSHIPS:

  • The Case Manager reports directly to the Aiyiikakimaop for ChildrenClinical Coordinator within Blood Tribe Social Development (BTSD);

 

  • Maintains a close working relationship with other Case Workers and other front-line service providers in BTSD andother departments and entities of the Blood Tribe.

 

  • Manages communication and consultation as appropriate among the clinical professionals and front-line service providers working with the individual and family to achieve integrated case management (e.g., through case conferencing).
  • Develops on-going relationships and protocols with critical Program partners as required. For example, the Kainai Board of Education and the Kainai Children’s’ Services Corporation.

 

AREA OF SUPERVISION:

 

  • Provides administrative supervision to clinical professionals in assessment and development of individualized MyCompass Plans, as well as in the provision of specialized therapeutic and developmental services specified in the MyCompass Plan.

 

  • Provides administrative supervision to BTSD and external service providers engaged through the AFC Program to work with the individual and family to implement the MyCompass Plan.

 

  • Trains and coaches other front-line service providers in the appropriate use of the MyCompass web-based application for case management and record-keeping.

 

MAJOR DUTIES AND RESPONSIBILITIES:

The Case Manager is responsible to:

 

  • Perform all duties and responsibilities in accordance with the Blood Tribe Administration Policies and Procedures and applicable policies of Blood Tribe Social Development.
  • Receive referrals from diverse sources, including self-referrals.
  • Accept referrals as approved by the Aiyiikakimaop for Children Program
  • Manage a caseload of individuals and families as assigned by the Aiyiikakimaop for Children Clinical Coordinator.
  • Develop relationships and protocols with critical partners as assigned by the Aiyiikakimaop for Children Clinical Coordinator.
  • Arrange for and administer assessments by suitably qualified professional clinicians, as recommended by the referring agency, at the direction of the Aiyiikakimaop for Children Clinical Coordinator
  • Work with the children, their parents / guardians, departments / agencies involved with the family and consulting professionals involved with the family to develop, document and communicate with respect to an individualized MyCompass Plan in the context of the family; as well as in amending the MyCompass Plan from time to time as required.
  • Conduct home visits as required to support the individual and family in development and implementation of the MyCompass Plan.
  • Maintain electronic records related to development, implementation, monitoring and amendment of the MyCompass Plan to enable and support integrated case management, primarily by using the MyCompass web-based application.
  • Organize the logistics of implementing the MyCompass Plan in support of the family and the individual.
  • Manage communication and consultation as appropriate among the clinical professionals and front-line service providers working with the individual and family to achieve integrated case management (e.g., through case conferencing);
  • Support the individual and family in navigating the system of service providers.
    • Attend staff meetings, periodic workshops and interagency meetings.
  • Assist in the orientation and training of new staff, staff of other departments and entities engaged in case management, allied professionals and external service providers.
  • Perform any other duties as assigned by the AFC Program Manager and/or Director of BTSD.

Knowledge and Skills

 

Working Knowledge

 

The Case Manager will have working knowledge of:

  • Social work and case management.
  • Available First Nation resources, education/training programs, government programs and services.
  • Social barriers, human behaviour, cultural diversity, family dynamics, and behaviour change.
  • First Nations/Aboriginal issues.
  • Child development
  • Current information systems applicable to case management implementation in the context of the Kainai Family Connections Initiative.
  • Disabilities and potential impacts on the individual and family.
  • Expertise in client assessment processes and procedures.
  • A variety of programs, policies, and procedures including, but not limited to programs and services of Blood Tribe Service Providers and Entities and Provincial Human Services programs,

 

Computer Skills

 

  • Windows platform, and MS Office (Word, Excel, PowerPoint, Outlook, etc.).
  • Effective use of computer systems for case management and fulfilment of reporting requirements.
  • Proficiency in the MyCompass application, sufficient to orient and coach other users.
  • Working knowledge of the BTSD Information Management system sufficient to ensure monitoring and evaluating program activity.

  • Must maintain confidentiality over personal issues and information.
  • Mediation, negotiation, decision-making, and facilitation skills.
  • Problem-solving skills.

The Case Manager will have:

 

  • Ability to work effectively in a First Nation environment.
  • Case management and Social program core competencies (ex. adaptability, client focus, communication, organizational awareness, problem solving and judgement, results orientation and teamwork).
  • Critical thinking and decision-making skills.
  • Knowledge of human behaviour theories.
  • Well-developed interpersonal communication skills, including appreciative inquiry and motivational interviewing, are essential to provide information, guidance and counselling to individuals.
  • Well-developed oral/written and active listening skills.
  • Ability to use of computer systems for case management and fulfilment of reporting requirements.
  • Good research skills.
  • Experience in providing case management services related to most of the following:
    • A high degree of independent decision making while applying professional standards to personal judgment and objectivity in dealing with clients and First Nation partners.
    • Sound relationship-building skills and team building skills.
    • Effective public relations.
    • Ability to assume a leadership role within assigned responsibilities.
    • De-escalation/ conflict resolution /negotiating/ troubleshooting skills.

Qualifications

NECESSARY TRAINING AND EXPERIENCE:

  • Must possess a minimum of a two-year Social Work Diploma or equivalent with three years related experience. A combination of Education and Experience may be considered.
  • Must possess a valid class 5 driver’s license and a reliable mode of transportation.
  • Must provide an updated criminal record and vulnerable check.
  • Ability to communicate effectively in the Blackfoot language would be considered an asset.
