GENERAL DESCRIPTION:

The AFC Clinical Coordinator is a senior supervisor who coordinates and takes an active role in the implementation of integrated case management for individuals and families receiving supports and services through the Aiyiikakimaop for Children (AFC) program of BTSD. Major duties include provision of clinical support and direction, clinicalsupervisionof a team of AFC  case managers; liaison with providers of clinical and community-based supports and services; and day-to-day supervision of the AFC case management system in accordance with the AFC policy and procedures. 

 

The Aiyiikakimaop for Children (AFC), a BTSD program designed to provide residents of the Blood Reserve under the age of 18 years with access to supports and services comparable to those available to all Albertans and tailored to the needs and culture of the Blood Tribe, by mobilizing and coordinating the necessary human, partnership and financial resources. The AFC Program was initiated in 2018 and is fully established.

AFC provides two streams of service:

  • Individualized programs of supports and services
  • Group respite initiatives  

 

ORGANIZATIONAL RELATIONSHIPS:

The Clinical Coordinator:

 

  • Reports directly to the AFC/AFA Program Manager within Blood Tribe Social Development (BTSD).

 

  • Provides clinical supervision and direction to the AFC case managers as they develop, monitor and refine MyCompass Plans for AFC children and families.

 

  • Manages the AFC caseload to ensure case manager staff resources are managed effectively and equitably.

 

  • Maintains a close working relationship departments and entities of the Blood Tribe, with a particular focus on referrals and integrated case management.

 

  • Manages communication and consultation as appropriate with organizations which provide clinical and community-based supports to AFC service recipients pursuant to contracts and standing offer arrangements.

 

  • Assists the AFC/AFA Program Manager in the various aspects of AFC program management.

 

AREA OF SUPERVISION:

  • Supervises (provides clinical oversight and direction for) a team of AFC case managers which averages six to eight individuals.

 

  • Authorizes and manages deployment of clinical professionals in assessment and development of individualized care plans, as well as in the provision of specialized therapeutic and developmental services specified in the care plan.

 

  • Authorizes deployment of clinical professionals and community-based Support Services Workers engaged to implement the MyCompass plans developed for AFC service recipients.

 

  • Brings discretionary decisions forward for discussion and approval to the AFA/AFA Program Manager.

 

MAJOR DUTIES AND RESPONSIBILITIES:

The Clinical Coordinator is responsible to:

 

  • Performs all duties and responsibilities in accordance with the Blood Tribe Administration Policies and Procedures and applicable policies of Blood Tribe Social Development.

 

  • Supervises the assigned group of AFC case managers from a clinical and therapeutic perspective.

 

  • Assesses (i.e., authorize, accept or decline) referrals for AFC services, based on criteria including:
    • Eligibility of the potential service recipient.
    • Whether the service requested is in scope.
    • Availability of qualified service providers.
    • Budget.

 

  • Coordinates all aspects of the AFC case management process for individualized programs of supports and services on including:
    • Provision of clinical expertise to ensure effective case planning and management:
    • Overseeing communication related to case conferencing and referrals.
    • Seeking direction from the AFC/AFA Program Manager on exceptions.
    • Participating in facilitation and documentation related to refinement of the integrated case management system in consultation with the AFC/AFA Program Manager.

 

  • Oversee all aspects of the design and implementation of AFC Group Respite Initiatives.

 

  • Oversees and coordinates record keeping (e.g., electronic records) and reporting related to development, implementation, monitoring and amendment of the MyCompass plans for the assigned caseload to enable and support integrated case management.

 

  • Coordinates relationships with service provider organizations engaged in providing the services of:
    • Professional clinicians (Psychologists, SLTs, OTs, PTs) contracted to provide:
      • Clinical assessment services.
      • Therapeutic and developmental professional clinical services as warranted by the clinical assessments and specified in the MyCompass plans.
    • Community-based support workers engaged in implementation of MyCompass plans.

 

  • Participates/supports in the recruitment, orientation and training of new AFC staff; as well as orientation and training of staff of other departments and entities engaged in case management, clinical professionals.

 

  • Supports the AFC Program, as requested as requested by the AFC/AFC Program Manager

 

  • Perform such other duties as may be assigned.

Computer Skills

  • Windows platform, and MS Office (Word, Excel, PowerPoint, Outlook, etc.).
  • Effective use of computer systems for case management and fulfilment of reporting requirements.
  • Competency in the organization and operation of video-conferencing applications and technologies adopted for use by AFC.
  • Working knowledge of the BTSD Information Management system sufficient to ensure monitoring and evaluating program activity.

 

Communication/Interpersonal Skills

  • Ability to communicate effectively in Blackfoot; ability to communicate in sign language an asset.
  • Public speaking. Comfort giving presentations to diverse audiences.

Qualifications

NECESSARY EDUCATION AND EXPERIENCE:

  • A minimum of a bachelor’s degree in social work or equivalent with a minimum of five years of directly related work experience is required. A combination of education and experience may be considered.
  • Knowledge of the AFC program experience in delivering social programs on the Blood Tribe/First Nation and knowledge of the Blackfoot culture and tradition.
  • Must submit an updated criminal record check and CPIC Check
  • Must possess a valid driver’s license and a reliable mode of transportation.
